I have attached a 3-page consultation sheet below. Please fill each page in using a desktop, tablet or laptop, rather than a smartphone. There are options at the top of the page to help you complete it. You can also attach pictures to the document (for blood test results, scans, skin issues, supplements, stool results etc).
The document will be automatically emailed back to me once it is completed by clicking ‘done’ in the top right-hand corner. Your consultation will last approx 45 minutes. I will video call you using Zoom at the time of your consultation. If you prefer a call on whatsapp, please let me know.
You will receive an email with personalised advice following your consultation. Payment is made on line prior to consultation.
